Conglutination
Conglutination is a completely physiological condition, when the stretching of a sufficiently wide foreskin of a penis is limited or impossible at a given moment.
Etiology[edit | edit source]
On the penis between the glans and the inner leaf of the foreskin remains sticking.
In conglutination, the limited overstretching is due only to the aforementioned adhesion and not to the narrowing of the peripheral part of the foreskin, as in the case of phimosis.
Therapy[edit | edit source]
Gradual release occurs spontaneously k postupnému uvolňování, ve většině případů není třeba aktivně zasahovat (Gradual release occurs spontaneously with age, in most cases there is no need to actively intervene (if there are no problems with urine outflow, etc.). Regular hygiene and gentle stretching of the foreskin is recommended.[1] Releasing with a tool (so-called dissolution) is not recommended these days. In the first days after the procedure, the area is very painful, therefore the smaller child often does not cooperate, which makes the necessary hygienic care impossible.[2] Residual conglutination can be disrupted at school age, if a small residue persists until puberty or adolescence, then it will be disrupted during the first sexual intercourse.
The idea that the foreskin must be free as soon as possible, so that hygiene is not neglected, is completely wrong. The part of the foreskin bag where the conglutination is present is closed sterilely, there is no chronic irritation of the epithelium and there is no need to worry about a carcinogenic effect.[3]
Complications[edit | edit source]
It is mainly related to dissection - formation of scarring phimosis, phimosis, re-adhesion.
