Patient case summary (in Czech hospitals called "epicrisis") is a summary of the course of the disease and the patient's health. The summary is issued by a doctor when a patient is discharged or regularly every day of the hospitalization. There is also a clinicopathological epicrisis that assesses the concordance between the clinical diagnosis and the pathological conclusion.
